Perennial herbaceous plant reaching 7–44 cm. Grows in fields and in forests.
There are 11 observations of Case's ladies'-tresses in Québec, mostly in Estrie, Outaouais and Laval, and it has been reported in 4 of the 17 regions. They peak in August and September.
